Dr. Theodore L. Aquino is a nationally recognized occupational medicine physician executive with experience leading medical operations for one of North America’s largest freight rail networks. As former Chief Medical Officer of BNSF Railway, he directed enterprise workforce health strategy for a safety-sensitive population, overseeing fitness-for-duty determinations, disability and return-to-work programs, drug and alcohol compliance, industrial hygiene, employee assistance program, and executive medical risk advisory across operations, legal, labor relations, safety, and human resources. Earlier in his career, Dr. Aquino served in senior medical leadership roles within UnitedHealth Group–Optum, where he led national case management, utilization review, provider credentialing, and large-scale clinical quality initiatives for the World Trade Center Health Program. His background also includes distinguished service as a U.S. Navy medical officer and department head, directing operational readiness and occupational health programs in complex, international, and high-risk environments. Dr. Aquino has published peer-reviewed academic papers, presented at national professional conferences, and provided deposition and trial testimony in matters involving occupational medicine, disability evaluation, and medical governance. His work reflects both scholarly engagement and practical expertise at the intersection of medicine, operations, and law. Dr. Aquino holds advanced degrees in public health and business administration and completed residency training in Occupational Medicine. He is board certified in Occupational Medicine and is a Fellow of both the American College of Occupational and Environmental Medicine and the American Board of Quality Assurance and Utilization Review Physicians. He volunteers as an Assistant Professor at Texas Christian University Burnett School of Medicine and as a Board Member of the American Heart Association – Tarrant County, where he previously chaired the Go Red for Women campaign, reflecting sustained leadership in academic medicine and service to the community. Through independent consulting, Dr. Aquino delivers objective, non-treating medical expertise in fitness-for-duty, disability and impairment evaluation, occupational causation, regulatory compliance (OSHA, ADA, DOT/FRA), workforce medical risk governance, and executive advisory services.
